About the Role
We are looking for a board-certified Podiatrist (DPM) to provide comprehensive mobile foot and ankle care to residents in skilled nursing facilities, assisted living communities, and independent living facilities throughout Florida. This is not routine nail care — you will deliver therapeutic, evidence-based care including diabetic foot exams, wound assessment, at-risk footcare, and MIPS quality reporting.
Why this opportunity is different: Active contracts in 250 Florida senior care facilities. Those are established DON and Administrator relationships; you walk into warm buildings with real patient volume from day one, not a cold territory.
What You'll Do
- Provide comprehensive foot and ankle care in SNF, AL, and IL settings
- Diabetic foot exams, wound assessment, debridement, nail and callus care
- Complete MIPS annual measures — Fall Risk #139 and Diabetic Neurosensory G8404
- Document every encounter in PointClickCare (PCC) — same day, signed before you leave
- Exit Conference with nursing at the end of every visit
- End of Day report to the Practice Manager before leaving the facility
- Manage ~25 patients per day across 1–2 facilities

- 250 active Florida facilities — day-one patient volume
What We're Looking For
Required
- DPM from an accredited podiatric medicine program
- Board certified — ABFAS or ABPM
- Active, unrestricted Florida DPM license (or ability to obtain)
- Active DEA registration, current NPI, Medicare enrollment
Preferred
- Prior SNF, AL, or LTC mobile practice experience
- PointClickCare experience a plus
- Familiarity with CMS LCD requirements and at-risk footcare
