About the Role
The Behavioral Health Team Lead is a licensed clinician who travels across an assigned regional territory to deliver clinical oversight, conduct behavioral health intakes, and lead a team of Behavioral Health Coordinators (BHCs) embedded in post-acute and skilled nursing facilities. This is a field-based role — you are on the road, visiting facilities, building relationships with clinical and facility leadership, and ensuring your region operates at a high standard. You serve as the clinical presence and escalation point across your territory, blending direct patient-facing work with team accountability and operational execution. Ideal for clinicians who want to get out from behind a desk and lead a growing behavioral health program across a defined region.
Key Responsibilities
Clinical Assessment & Intakes
• Conduct behavioral health intakes and comprehensive psychosocial assessments
• Perform diagnostic clarification and risk stratification
• Determine program eligibility and level of intervention
• Engage patients and families as clinically appropriate
Care Coordination
• Collaborate with psychiatry, psychology, and medical providers
• Participate in interdisciplinary care planning
• Support transitions of care and discharge planning
• Provide clinical guidance and escalation support to BHCs
• Review documentation for quality, compliance, and consistency
Regional Travel & Facility Management
• Travel regularly across your assigned region to visit post-acute and skilled nursing facilities
• Build and maintain working relationships with facility administrators, DONs, and clinical leadership
• Serve as the on-site clinical presence and escalation point across the territory
• Ensure consistent program execution and team performance across all facilities in region
Qualifications
Required
• Active, unrestricted LCSW, LMHC, or LMFT license
• Master's degree in Social Work, Counseling, Psychology, or related field
• Medicare-eligible for intake and assessment services
• 3+ years behavioral health or case management experience
• Ability to travel regionally
Preferred
• Post-acute, SNF, or geriatric experience
• Value-Based Care or BHI program experience
• Prior team lead or supervisory experience
• Bilingual a plus
